SIFA is an offshoot of the Maine Community Health Needs Assessment project in 2024 and 2025

SIFA goals were developed by a working group of Swan's Island residents

The SIFA concept was inspired by the successful Monhegan Island "Doc on a Rock" program

SIFA will be an independent non-profit organization which will work in a collaborative relationship with the Town of Swan's Island
